I'm glad they didn't get into the science - while this is a 'sci-fi' thriller, it's not a science movie - it's more focused on the plot than the semantics, which was a great decision. I don't care how it works, as long as it does, which in this case it did.
Only thing that has me confused is the whole limbo thing...why didn't they just shoot each other right away to get out of limbo? That's the only thing that I can't 100% understand, but otherwise this movie was well written and kept you thinking the whole time.
Awesome!
I've been trying to figure this out too, this is what I have put together.
[SPOILER]
There's two ways to get into limbo right? Going deep into the levels by dreams or by dieing in one of the dreams while being sedated. Since Cobb and Adrianne went into limbo via dream(to save Fischer) in the snow fort level they KNEW they were in limbo, but if you die in one of the levels and get sent into limbo you get "washed up on the shore of your own subconscious" not knowing how you arrived there and in a state of confusion. A state of confusion that is comparable to a dream, when you're dreaming you don't know how you got there, but your subconscious fills the space making you believe it's reality, so when you get into limbo via death you assume it's reality.
This is why when Cobb is killed by Mal in limbo he is washed up on the shore of limbo in that state of confusion and he looks confused/lost when he is sat in front of old Kaito. Once Cobb realizes this could be a dream he pulls out his totem(the top), this is when Kaito begins to remember about the plan(since he didn't have a totem he thought limbo was reality) and they start to share dialogue and then says "let's go be young together". At this point I believe that Kaito shoots Cobb and then himself.
Or maybe I have it all wrong.

Awesome movie.
