Anyway, have you checked injectors? Try this, when the car warms up, leave it in idle and start disconnecting the electical harness to each injector. One by one, reconnecting as you go. If you see a change in the idle, then the injector is probably fine, if you remove a harness from one, and it doesn't change the idle, you may need to replace that injector or check the electric curren to it.
Also, you say you've had this car since '96 w/13k miles? If so, and by looking at the obvious sludge, I assume you've never changed the o2 sensor like the dealer told you? You have to change it every 60k miles or so. What about oil filter/air filter, and the like? I'm not trying to patronize you or anything, but it's just common procedure. You can try to clean the o2 sensor w/some mineral spirits, and see if there's a change.... although it may not be safe for it. That rust looking spot on the cam is worrying. There should be no water to oxidize it. Indication of water in the oil usually means a blown head gasket. Check comp.
