I just replaced the timing belt on my '87 Maxima. Instead of counting the teeth in the old belt between each of the three gears before I removed the old belt, I only lined up each gear with its timing mark, expecting that the alignment marks on the new belt were going to line up the timing marks on the gears. Well, they didn't, and when I put the new belt on, it was a half tooth off on one gear, probably because the new belt wasn't stretched yet. When I cranked the engine back up, my timing was off pretty bad, but adjusting the distributor fixed it, although I had to turn it almost to one extreme to get it. It still doesn't seem to run quite the same as before, and considering how much I had to adjust the distributor, I suspect I'm one tooth off on the belt and wish I had counted the belt teeth between the gears.Not sure if your problem is more major than this, but maybe this will help...

No counting needed really, just go by the marks on the belt itself, those are accurate. The dimples on the back cover that you align the cam gear dimples with are solely there to get in close to the correct position, you then line them up exactly with the marks on the belt.

Distributor wise, if your timing belt is on correctly and you have to crank the distributor far one way you're a tooth off on the distributor. If you're a tooth off on the timing belt I'd fix that asap as the VG is an interference engine, the valves can hit the pistons which means, at the very least, a new head.
