i had a local shop put some mickeys on the rear for me once, like swapped out the tires...didnt tighten the lugs up on one side all the way...like 10 minutes down the road the truck started to sway i stuck my head out the window and the wheel/tire were about to flop right off. i stopped and fixed it but the inside of the m/t's lug holes looked just like yours. like almost ground out inside. was pissed.
maybe they were loose, or loosening as you drove and got to that breaking point in that final few seconds and off came the wheel?
You sure you werent just excited about it and forgot to tighten them? Happened to me after I did my coils but I noticed 50 feet from my house and saved them.

well, look at it this way, weather is a 95 or a 04, ur goin to dump money into it anyways so why not just fix the one u got now instead of dumping money onto a car that u have to make payments on... besides thats what make ur car truly urs.. u fixing whats wrong wit it n making it better/ or your own..
