I think running rich is the obvious, it's WHY he's running rich we're concered with thus the cold start mode problem. I would definately check/clean that IACV, replace coolant temp sensor or at least check the connection for corrosion. Do the same with O2 sensor/MAF. Clean the TB while you're at it.
I don't know about the KA, but the VG30DE will spit 4x the normal amount of fuel per combustion cycle when in cold start mode.

Do the math. I believe it does that to warm up the motor efficiently and get those cats doing their jobs for the environment quicker.
