Where can I get an armrest add-on?

I just did a long trip and figured add something on top of the armrest to support my elbow completely released the pain of long driving. This is to simulate the armrest you can normally find in a minivan.

Where can I find something which can nicely fit on top of the existing door panel armrest and mid console armrest?
