whats wrong with my s14?

when im going and change gears (from any gear) its feels like i put from like 1 to 5 it just studders until it gets to 2500 rpm and the transmission or somewhere around in there sounds like its making a clacking sound anyone got any advice as to what it is? ... and also down were the shifter is it sounds like a kind of slight grinding noise (have the bezel off and boot off) forgot to mention completly stock
