The line coming from the valve cover is a breather line, you need to run this to a catch-can or put a filter on it.
The large line coming from under the manifold should be the IACV line, all it needs is a METERED source of air. Meaning plumb it anywhere into the plumbing after the MAF.
The smaller hose (3/8" ?) is more than likely your evaporative emissions dump line. It will be going to the charcoal canister. You can just run this open, or back into a negative pressure to manifold line. I would just eliminate the evap system all together.
