Did the same mechanic do all of your rotor replacements? Although excessive heat is what's normally behind a warped rotor, there are a few ways that heat can be generated, so it's tough to point the fickle finger of fate without more info.
The reason I brought up the mechanic is that it's possible that in one or more of the cases, he might not have properly diagnosed the real cause or perhaps did not do the repair properly. Think about it, if something else is causing the rotors to keep warping, like improperly adjusted calipers for example, then replacing rotors would be a temporary fix.
My recommendation would be to not only replace the pads/warped rotors, but to have your entire braking system checked out. Perhaps by someone other than the guy who did the previous jobs. Then immediately "bed in" your new pads according to the pad manufacturer's recommendation. Bedding the pads involves a series of highspeed brake applications, (preferably on a deserted highway) in order to wear in the new pads down to exactly match your rotor's entire rotor surface. That maximizes your braking power and levels out the hotspots, reducing the risk of rotor warpage. Very few people actually do that but it actually does help.
