Urgent! Replace CVT @ 199,000 km? Can't maintain steady 30 kph

2007 Max SE 199,200 km. CVT Unsteady at low speeds. Can't maintain school zone speeds.
Need CVT Replaced?
Very critical time as my CVT has 800 km extended warranty left. Suggestions?

About 15,000 km back I thought my power steering was starting to whine. Serviced, added new fluid and conditioner -no change. Checked more closely, stethoscope on power steering -no whine. Stethoscope on CVT and hear definite whine. Can hear whine change in pitch/volume slightly when shifting from neutral to Reverse or Drive. Whine is louder in hotter weather or after highway driving. A few cold starts, I have had nothing, no shift, when shifting into R or D from cold start. Repeat shift a couple times and all works again, but seems very slow to engage.

Beginning about the same time, I can no longer hold a steady 25 or 30 kph in school zones. CVT seems to coast then re-engage. Speed cycles uncomfortably from 25-35 kph as I fight to hold it steady. It feels like there is wear spot? Steel belt wear? At the lower speed it seems the CVT cannot hold a steady, engaged gear ratio? Above 35 kph works fine except for all-speed, noticeable whine. Whine definitely sounds like a pump on its way out to me.

Fluid level and color okay. Has been serviced, but never flushed. No leaks or other problems.

Would like to go to 300,000 km+ with this car. Brand new 120,000 km warranty on pricey, smooth, quiet Nokian All-Season tires :)
