She was stern but polite and it was my fault, so I obliged her refund. I'm baffled by the purple text and the cartoon image of the boy sitting on the floor. Is she using the image to tell me she's upset?

The rest of the email is just talking about the book being inaccurately described, nothing unusual. Good grammar and spelling.

I may as well paste it:

To whom it may concern,

I had previously made a purchase for the medical terminology book, to include the CD and the actual book, and the flash cards. To my surprise I only received the flashcards, which was contrary to the description made with the product. I would appreciate it if you would contact me as soon as possible because I would love if I could get what I initially paid for. If you are not going to fully hold up to your end of the deal, then I do want a full refund.

All of the online transaction companies are about the consumer. Even in top rated software for your point of sale system, if a customer disputes a transaction, the merchant is for the consumer.

No one likes giving refunds.

I think it's simply a misguided attempt on her part to reinforce her displeasure in what she thought was a cute way. the pic can be interpreted a few ways: a fallen boy is either upset because it hurt or angry for falling. Obviously she's not happy with this particular transaction.

Since she seemed to have a point, it sounds like you fixed it. So that should be the end of your transaction with her. However, if her claim was baseless, then you could have some fun and reinforce your decline message. For example, if you want to tell her to blow it out her azz, then this could work...
