I just put new 20's on my 03 altima and it rides pretty rough now. What type of suspension or other upgrade can I put on to smooth it out?

18's would be a good place to start. PS and welcome to the club.

Is the rest of the suspension stock?

You could almost maybe go with softer springs and really good shocks... But you may run out of suspension travel and it will be even harsher if your hitting bump stops...

Bottom line 20s are huge wheels, tires absorb a lot of stuff and now that you got rid of most of your tire you are probably going to have to just deal with it.
