I dont have a color chart with me, but basically, you just need to buy a Premium Line out put converter. Normally about $20. It has speaker wire input, and RCA outputs. Wire this to your rear speaker wire behind the radio, then just run RCAs to Amp like you would for a normal aftermarket radio.

What Kolk1 said is true, or, if your amp supports high level input, you could just run signal wires directly to those inputs. Either way, you'll end up running a 4 or 8 gauge wire directly from the battery (with a fuse as close to the battery as possible) to the amplifier and another wire of the same gauge connected to the car's chassis. Then run signal wires from the stereo and speaker wires from the amp to the speakers and you're all set.

Expect to take a good day or so to run all the wires properly hidden behind plastic and under carpet. If you need help figuring out wire colors, you should download the shop manual (available linked from somewhere on this site, do a little searching.) There is also a good how-to on replacing the stereo that has good info that could help with this project.

Hope it goes well.

If you are using RCA you will need an adaptor. I usually buy things like that online, but you could probably get one at any good car electonics stores or possibly even at someplace like best buy.

I've had success with the PAC AOEM-NIS2 unit. They cost a bit more than some of the others but it's a quality unit.
