How did you come to the conclusion that it needs to be replaced? About the only reason I can think of is if the head is cracked from overheating. If this is the case, you'll be replacing more than just the head.
But to answer your question; - unbolt exhaust manifold on driver side. - unbolt intake manifold from passenger side. There are several bolts down by the injectors and 2 really long bolts to the outside of the manifold as well as a bracket underneath that bolts to the block. - unbolt PS pump bracket at the front of the head. - unplug electrical harness connections - remove valve cover - set #1 cylinder to Top-Dead-Center - unbolt Cam Timing Gear and remove chain - unbolt Head Bolts - remove head - replace head gasket - replace intake and exhaust gaskets
Always have a used head checked thoroughly at a machine shop for cracks and to make sure it isn't warped and the valve seals are in good condition.
You probably want to get some white label zip lock bags to put bolts in and label where they came from.
