Cold crank and warm [including ATF warm] is just that, the period when everything is unstable.
We usually find dirt in intake path and used dirty spark plugs and injectors.
Many time the fuel is not adequate for the situation nor is a suitable warm period utilized before driving away.
As you say after 5-10 minutes things seem ok whenever the coolant and ATF reach required operating temperature.
Ethanol does not vaporize well cold, nor does much of US gasoline [especially during the October-November] transistion to winter blend] so it has to circulate thru the engine comparment to warm up what in the tank. in severe cold conditions.
