What a testament to the durability of the truck though... 155,000 miles and still on the road after 7 years of total neglect and abuse of what appears to be an extreme nature.
Just take one thing at a time and you get it going right.
Sounds like your going to start with the exhaust which is a good idea.
Get those codes pulled and fix those as they absolutely need to be taken care of to get your truck running smooth.
Might I suggest that you change as many fluids as possible as soon as possible. Something tells me that this woman wasn't really conscientious about preventative maintenance if she didn't even fix things that were obviously wrong.
As far as the engine problems, try a BG44K fuel treatment. You can get it on eBay for about $12/can shipped. The only thing close is Techron fuel treatment but even that is not as strong / effective. It really has no comparison. You just add it to the gas tank. BG44K will really clean out the whole fuel system. You'd be suprised at what it can do. M45s are notorious for needing to get the throttle bodies cleaned otherwise you get unresponsive and sluggish / studdering engines and BG44K is the king of fixing a lot of these problems. Go through a couple tanks of gas and then do an oil / filter change.
Then change the fuel filter. (easy)
Beyond that, I would change the rear and front differential gear oil, transmission drain and refill, 4X4 transfer case fluid change, a coolant flush, brake fluid flush, and a power steering fluid flush. It sounds like a lot, but it'll keep your truck on the road. You need to turn this truck's life around. Most of this can be done by you as long as you have a little mechanical ability.
