My 92' Q45 runs hot when I hit taffic regardless of the temp outside (I live in the SF Bay Area where the nights are mid-40s). Where should I start, with a new thermostat?

Thermostat would be a high speed/load problem. Check both fans, clutch on the engine fan, motor on the electric one.

If the radiator is more than 7 years old it will be down in performance by 20% at minimum!

The t-stat isn't out of the question though. If your t-stat is sticking you will see an increase in temp at idle. If that upper hose is hot to touch your looking at the t-stat. But like 911/Q45 said that fan clutch has been known to have it's problems.
