Reasons for Code 45 (injector Leak) besides bad injector?

Ive been dealing with this problem for many years and after a 3rd plenum pull with no solution im stumped. This time i replaced all the injectors plus all the seals and still im getting a code 45. Also all the connectors for fuel injectors and O2 sensors have been replaces. Plus O2 sensors (oem bosch) and all the fuel lines.

The code 45 is not automatic it will only happen if i keep the accelerated pedal pressed to keep car running at constant speed.
Car is running rich, air fuel base is at 109%.

What are some other reasons for a code 45, at this point even guesses are welcomed. Thanks.

Sounds like the computer is seeing a lean condition and making it rich to compensate. If so there could be an intake leak. I've also seen newer nissans do this due to bad readings from the MAF sensor, tricking the computer to think it's running lean when it's not.

Ill check for intake leaks. When it comes to the MAF the voltage at idle is 1.27v which is within spec (0.8-1.5v).
