please help me help my friend

my friend told me the other day that his slave cylinder blew out. He had a friend replace it (the pedal has pressure) and now his car will not go into gear and it dies when he pushes the clutch in. He says "Because the car will start, you push the pedal in to put it in gear and it dies, so you have to rev the car and hold it at like 1200rpms to put it in gear, when you're moving, shifting gears, the gears stick and grind. The pressure plate won't release and when the motor turns you hear something in the transmission area grinding"

is the shift fork loose in the bell housing?

UPDATE : It was the throwout bearing... it was demolished! High Rpm shifting and just hard shifting caused it to happen... i dont know why the slave cylinder blew though.. thanks
