Unless it's directly causing any trouble, it's not that important. I'd replace it as part of a maintenance routine when working around that area.
It depends on what you consider being "worth the time and effort." If the pcv fail and cause all sorts of madness - idling issues, stumbling, etc. It may be hard to trace it down to the PCV (if it's the cause) bcs there are so many diff things that could be it.
If you replace it before it goes bad, then you have the peace of mind that it should be fine for another 100k+ miles.
