what exactly does this do? would it improve gas mileage/ignition without a msd ignition system of some sort?

It would be better to use it with an msd unit but you can with out it. You really don't need to worry about it since you have an internal coil and it won't work....unless you have found a way to convert obd2 ignition system to an external coil. But anywho an ignition upgrade would benifit in gasmilage smoother throttle response, etc.
