I am wanting to swap a SYNC system into my 1991 Q45. Anyone know how big of a job I am looking at?

Looks like they run ~$300 @ http://www.car-part.com/

I'm going to guess that it's not that easy. Your going to need to install a mic to it so that i knows what your saying. I also THINK that there is probably a steering wheel interface that signals it to listen for voice commands, and you'll have to figure out the appropriate resistance of it and what wires it connects to. There might be more to it, or their might not. The only good way that I know to figure it out would be diving into the fsm for the particular car and start dissecting it on that level.
