Sounds like timing chain rattle, which is odd for 183k miles. Usually that doesn't set in until the 220k+ mark. But my guess is timing chain rattle.

So if it is, how hard is it to replace them myself?

I would first start by checking your oil. The noise could simply be the result of low or very old/dirty oil.

Timing chain replacement is a pretty big job, especially in the cramped Maxima engine compartment. It can be done, but it's definitely a time-consuming job that I would only recommend to someone who is very comfortable with their mechanic abilities. If you screw it up, you could make things a lot worse. That said, if you think you can do it, then by all means save yourself the labor costs and try it yourself.

I just changed my oil last tues i put in Castrol GTX synthtic blend and its been running better but that noise is still there. I'll probably just take it to nissan and ask some questions and figure out what to do from there. Thanks for the advice!

Try some LUCAS Addetive. Works really good.
