Loud rattle at idle

I have a 91 nissan 240sx from idle till around 2700-3000rpm there is a severe rattle coming from motor. i had a timing issue at one point and when it was retimed the balancer shaft was not timed correctly that is when the noise started. a mechanic pointed out the problem and when i retimed the balancer shaft the noise went away only to return about a month later only worse. i checked the timing again thinking perhaps it had jumped a tooth but it is timed correctly. any ideas would be appreciated.

Rod knoock? The higher RPMs make higher oil pressure therefore, the knock goes away. I'd be willing to bet that's what it is.
