You have to pull the head at the very least. If the pistons have been damaged, there will be telltale scrapes on the cylinder walls. I've also seen where blown head gaskets leaked coolant into the bores and pitted the cylinder walls.
Damaged pistons can sometimes be hard to tell from a top down visual. Most times, overboosted motors will have pistons that look like they were burned on one side, and in the blackened areas, you'll see where the metal was starting to bubble and melt.
Some damaged pistons, however, have cracked ring lands and are impossible to tell with them still in the block. If the ring lands start to chunk up, they can scrape the cylinder wall, giving you a hint that something is wrong, but I had 3 pistons in my rebuild motor have cracked/destroyed ring lands, but they didn't scrape the walls up. One actually heated the ring land up so much, it vaporized and only left a blue mark on the cylinder wall.
All of that happened from the previous owner cranking the boost up too high without upgrading the fuel management.
You can see where the ring land melted out and went out the exhaust.
The rest of the ringlands starting to fall apart when I deringed the piston.
