J30 Crankshaft pulley walked out

Ive been having alternator belt squeaking issues for a very long time, and now the PS belt just had snapped. Upon closer inspection, it seems that the outer part of the two piece crank pulley walked itself over the power steering pulley (towards the fan/front of car) about 1/8-1/4 of an inch. This causes the alternator belt to squeak a little, as well as preventing the power steering belt from properly being installed at all (totally bent, flew off after 5 min riding)

Belt tension hasn't caused this problem, because the alternator belt is now crooked.

Has anyone else have this happen to them?What could of been the cause?

I plan on replacing the pulley, and hope that it will solve the problem for many years to come.
