If you have either an iphone, ipod touch or blackberry storm, there's a fun little app called Dynolicious that utilizes the phone's built-in accelerometers to do all kinds of neat stuff! (Though it's not terribly accurate.)
As for an app? That would be cool and I'd be willing to throw some time into it... however, I just turn any car-related docs into PDF files and put them on my phones. (Have a BB Bold and iPhone 3GS 32GB)
I personally use Adobe Acrobat, but there's a neat (free) little PDF creator called PrimoPDF. Or, if you already have OpenOffice installed, you can copy the info into a document and then save it as a PDF. (Same with Office 2007 as well!)
TBH, I've been considering creating a web-based "garage" app. There are similar websites out there that let you create a garage and then add your car, and then you can log maintenance performed. However, they're not very streamlined or phone-friendly in my experience, nor can you link maintenance docs, notes, etc.
Now that both the iPhone and Blackberries can use web-interfaced apps, I'm strongly considering it. The problem is that time is money (being that I'm self-employed), and if I can't be sure I'd make enough money off the app/service I'd be left high and dry...
