Idle problem but only when something is "sputtering" underneath car

My idle drops down to about 500 and the whole car sounds like crap when there is a sputtering sound underneath the car. Is it an exhaust leak? I dont know what would cause the idle to drop like that? Possible O2 sensor? I don't know where the sound is coming from. Any ideas?
