The problem is the engine requires more air during cold start and cold running conditions then during hot conditions. So if you set the throttle body open to perfect idle while warm, it will not idle while cold. Likewise if you set it for cold idle, it will idle too high when hot.
Not to mention if you set it "perfect" during warm idle, any slight variation in temp/conditions will cause the idle to go up or down from there. So it will never be "set" correctly.
Run a proper IACV, Run the coolant through it too. without coolant the idle with be too high because the warm coolant causes some wax compound to expand (iirc) in the IACV which lowers idle. I think the TB also gets coolant, but it has to do with the temp of the TB not the idle speed, iirc.
