help help my transmission is acting like a JERK

ok so i have a 92 sr motor with all the basic mods. i have a 13lb flywheel and a exedy organic clutch. recently i guess over the last two months i have been having random troubles getting my car into gear while it idles and then sometimes in stop and go traffic it will not like to come out of first or second gear. shifting isnt always as smooth as id like and i have had the clutch in since last november and it was great but now it seems like it slowly starting to go in to suck mode.

ALSO BIGGER PROBLEM MAYBE IT HAPPENS ATLEAST ONCE A DAY. SOMETIMES WHEN IM AT A LIGHT ILL HAVE THE CAR IN FIRST GEAR WITH MY FOOT ON THE CLUTCH AND ULL NOTICE THAT MY CAR STARTS TO PUSH PAST THE CLUTCH AND STARTS SLOWLY INCHING FORWARD. MIND YOU MY CLUTCH IS ALL THE WAY ENGAGED.

any ideas or people with similar problems what do i do to fix.
