havin more problems

is the cam chain tensioner sitting between the cams or a ka24de a part the has to be there? and my car is makin a bad noise. it used to be just lifters tappin pretty hard. but its turned from a tap to just a complete clatter non stop til u get to high rpms ne ideas? could a valve gone bad? or a lifter be completely fried?

the chain tensioner is on the passanger side of the engine(or intake side), at the top and exhaust side there's chain guides, most people remove'em, nissan had a tsb(i think) about that so they removed the top 1.
