Would it be better to rev up to speed really quick, go into 4 or 5 rpms, or would it be better for mpg to slowly rev up.

I've always heard slowly, but I figured I'd check.

The obvious answer would seem to be accelerate slowly. However, any speed under about 35mph, will result in lower fuel economy. I think it would be best to accelerate using about 3,000-3,500 (where the engine is closer to it's torque peak and should be most effecient) up to 35-40mph then ease off. THe most improtant thing for economy is to use the brakes as little as possible, so anticipating when you will need to slow down, or taking routes that have fewer stop signs.

I think the rule of thumb for good gas millage is accelerate like you have an egg between your shoe and gas pedal.

Well it would have to be hard boiled of course. I like to drive as though I have my puppy's head between my foot and the gas pedal. But that's just me.

I tend to drive like the gas pedal is my arch nemesis' face. But that's just me.

It's not so much how quickly you rev the car as it is how high you rev it.Every engine has an ideal point for economical shifting. Stay below X revs, and you'll do better. How quickly you get there isn't really a big issue.
