Fuel injector failure

Does anyone know what would make brand new injectors go out? A few months ago my brother took his car to the dealership and they said the fuel injectors were leaking and we were getting the codes Po171 and Po174 that both cylinder banks are running lean. So we replaced the injectors and after only maybe 400 miles we were getting the same codes and took it back to the dealership and they’re saying the injectors are leaking again. The guy at the dealership said the fuel samples look good so does anyone know what might cause this?

I wonder if whoever did the job was inexperienced and put the new ones in without lube? Or maybe they hosed you and put in reman or aftermarket? Brand new OE injectors rarely fail like that.

As stated above, could have been installation issue. I keep hearing of new parts failures being on the rise for all products so it could simply be shoddy manufacturing
