Is this a sham or an actual worthwhile thing to throw into the gas tank every so often.

Depends if you are referring to quality products [BG44k or techron] or the usual things you find at stores.

Even the BG44k won't clean really dirty injectors fast it may take 2-3 treatments or removing them and ultrasonic cleaning.

The Power Balance test using the Consult will tell you that all 4,6,8 cylinders are not balanced usually caused by injectors.

All in all a 30 minute BG rail flush does a good job if youy don't want to remove and clean.
