Fuel and a missing issue

I have a 91 240, and im having some issues. I cant shift to 5th unless im goimg 70ish, if I shift before that its at 2.5 rpm and bogs and sucks gas, so I ride in 4th everywhere I go even hwy. It also sounds like the rpms are higher than its reading on my gauge, how can verfy this? It also misses sometimes when im riding it and the rpm will jump from 1 to 2.5ish when im stopped. Can someone please help me with these issues, thank you for your time.
