Considering I just replaced the speakers not to long ago... I actually forgot the exact details... but i'll try to remember as much as possible.
I did this in a 99.5 (99 late model) PF, so I'm not sure how similar it is to the 95 model.
first!!! remove the piece of trim around the door handle. pry the front (the half closer to the front of the SUV) part first because the back part has a piece that is easily broken (because that part slides into position to hold the plastic trim, where as the rest have little tabs that snap in). once you pop the front half, slide it towards the back (along the door). i've broken the plastic tab on two of the four doors because I didn't know the first time and then forgot when I got around to doing it again later. you don't need to touch the door handle itself.
going down the door, under the handle, is the pocket that you usually grab onto to close the door. in that pocket is a small piece of removable plastic. under that plastic is a screw you'll need to remove.
then you have to pop off the armrest part. Those are held in by some very tight/strong clips, so don't worry about breaking them (though you could scratch visible parts of the panel trying to separate them like i did).they pop up along the door. it might be one piece or two pieces (the second can only come off after the front piece comes off). you'll have to disconnect the wires from the switches once that part of the armrest comes off.
once you pop those off, you'll see two or three bolts. you probably could use a ratchet, or just a regular Philips screwdriver. after that look to see if there's another spot that looks like a plastic cover for a screw hole, it'll be bigger than the one in the handle pocket.
Once those are removed, you'll have to deal with all the plastic tabs (runs along the edge) that hold the panel in place... i suggest you get a butter knife. there's actually a tool specifically for this, looks alike a flat tuning fork (like this). find the loosest section of the door panel so you can see a little bit. side the butter knife in, and then slide it right up to the tabs. pull the door panel and turn the knife so that it's acting like a lever to pull the tab. the reason you want to do this, is because if you don't use something to pull on the tab, the tabs can come off the panel. while that isn't a big deal in itself, the panel is made of soft ply wood underneath... which damages easily (like a tab coming off). you'll see what I mean once you're done... I messed up a few times, so I know what happens. AGAIN, this is based on the 99.5 model, you might not even have plywood back there or even the tabs (i don't know).
and that should do it for the door panel. all of it is true for front and back doors.
