Toyotas brake problem was largely attributed to operator error. Lets bear in mind that that this comes from the same people who have so many floor mats or floor mats in such a position that they impede the operation of the brakes. If you have multiple floor mats running loose on the drivers side of your car shame on you and please remove all but one and secure it for the benefit of my family.
They have unintended acceleration and acceleration despite brake input as part of a software flaw. Nissan has had a failsafe function that will cut the throttle if both pedals are depressed. (try to power brake the car what happens???)

please let us know how you made out with the fix... my sister in law's vehicle is behaving much the same way (stalls out while accelerating or decelerating - shuts off and restarts immediately thereafter). Her safety is being compromised. The dealer behaving as you'd expect... a lot of head scratching and too cheap to aggressively tackle the issue under warranty.

The dealer is not being cheap. They just are not allowed to replace parts they cannot prove are failed. If the warranty claim gets kicked back than the dealer has to pay the entire claim.
I can tell you guys I have seen the other end of this it gets very frustrating if we cannot actually find a cause but if there are no codes and the problem cannot be duplicated (no matter how severe at the time) there is really only so much we can do. If the problem reproduces itself or triggers and one of the 100+ trouble codes throughout every system of the vehicle it is something that would point the technician in a better direction.
Random loss of power should most likely trigger a fault. A stall should trigger something somewhere. However I have been watching this forum since it started and I have only seen a small handful of similar complaints. The probability of a widespread issue is a not really something I would stress over, life is far to short.
Note- All repair facilities and all technicians are different, in a perfect world the repairs would be more standardized and there would be perfect quality control.
