I want to build a custom Cold air intake for my 2001 maxima. for anyone that has build one, where did you connect the crankcase breather hose?

Drill and weld in a port for it behind the MAF.

i have three hoses, a large, medium, and small. the small is a vacum, and the large one is the breather hose, what is the meduim hose? should i drill and weld ports for all three hoses?

Yes, it is best to re-attach all the hoses.

one last question before i set out on my mission. should i drill and mount the hoses on my new intake tube (closer to my filter) or right outside my throttle body (closer to my throttle body, before the MAF sensor)

IIRC all of the hoses connector after the MAF but before the TB on the stock airbox setup.

You should replicate this.
