Clutch question with video

I started to notice some occasional vibration in my clutch pedal along with a growing tendency not to return all the way out. I was advised to replace the slave cylinder, so I did, and the problem continued. I tried to change my engagement point and would either get it to not vibrate and come all the way out at the expense of having the engagement point exceeding the clutch pedal's outward travel as the car sat out in the sun for a while. I then tried to adjust it so that the hydraulic return met up where the spring takes over hoping it would fix the problem and my vibration came back. I was sitting still so I jumped out to take a look. I was a little shocked by what I found going on at the slave cylinder so I whipped out the camera to take a quick video.

If it wasn't obvious this is parked with the clutch pedal all the way out.

With the clutch fully depressed the clacking sound and vibrations all go away

Is this:
a) a matter of improper clutch engagement point
b) damaged throw out bearing(clearly something wrong with that)
c) Misshapen clutch fork
d) still something wrong with the hydraulics
e) A combination of the aforementioned ideas
f) Something I haven't thought of yet

Feel free to point out any silly assumptions or severe oversights that I have made.

Another video taken at the same time. You can see the vibrations from the previous video translated to the pedal and when the cam goes under the car you have a clearer sound of the clacking that is associated with the vibrations. Also, I don't know what that sound was a 0:06. I hope this second view can help.

i would guess, but am not positive, your pressure plate is broken. I can tell from the video its a mechanical problem inside the bell-housing so your going to have to take out the transmission either way.
