Clutch Issues

Sorta stumped and looking for answers.

I had a throw-out bearing go bad about 2 weeks back. Bought a new OEM clutch kit to replace & brought it to a local shop. Just before putting it in the shop the slave cylinder went bad as well (piston full extension).
They said they checked the fork and it didn't seem to be bent but I wasn't able to confirm this. After replacing all necessary parts the clutch was not disengaging. They took another look at things and replaced the ball that the fork sits on, slightly worn. The worked fine for a few minutes worth of driving then the clutch wouldn't disengage and the slave cylinder went bad again (piston full extension).
They then determined I had a bad Clutch dampener. I ran a bypass and had them replace the cylinder and the slave went bad once more, same symptoms.
My hypothesis is since the the throw-out bearing went bad, it caused the pivot fork to bend slightly due to the added pressure needed to push the worn bearing, which in turn cause the dampener to go bad. The only original parts are the clutch fork and the clutch pedal...and lines. All other pieces are new additions.
All in all I believe the pitch fork is bent. is this possible??

Shouldn't that kinda drastic bending be noticable, even though the slave cylinder rod is still making contact with the fork?

Could this maybe be a clutch pedal re-adjustment issue. Before the initial fix the clutch would engage at the top ot the pedal. It was re-adjusted to grab at the bottom. with the past two slaves in stalled. But then again it the slave pushed out when it initially broke as well.

you might have broke or worn the little nipple thing for the fork to pivot on
