Clutch Feels "Gritty" Anyone know whats going on?

Here is the issue. This usually happens once the car is warmed up and i've been driving for a few miles. The clutch feels "gritty" usually when I press the clutch to get from 3rd to 4th. Also if I drive hard, i've had the shifter pop out of gear once I press the clutch and my foot reaches then engagement/disengagement point. Anyone have any clear answers to this problem. Also my throwout bearing makes noise when in neutral and the clutch is disengaged, this has been a problem since I installed the 25det transmission and the car drove just fine. Specs are below. Thanks.

S14 - RB25DET w/RB25DET Transmission
ACT HDG6 6 Puck Clutch (20K miles)
Stock RB25 Slave Cylinder
Stainless Steel Clutch Line
Stock Flywheel
Aluminum driveshaft
Stock Shifter

I can't think of anything else that could be related to the issue.
VLSD with not much life left....

Sounds like your throwout bearing is toast.
