car hesitation

my car has an erratic idle, drops from 1000 rpm to about 400, sometimes stalls. also, when im driving down the street at a constant speed, the car jerks everyonce and a while, and sometimes when i floor the gas pedal the car hesitates to accelerate. what could be the problem? any suggestions? do i have some kind of vaccume leak in one of the hoses, bad cams, or is it just plaing running too rich?
