Usually it refers to a cracked or warped head. This usually occurs because of excessive heat. It could also mean a blow head gasket, I suppose. This means you literally spit the head gasket out of place.... or fried it.

When you have a 'blown head' or gasket, you will lose compression and experience power loss. This will also usually lead to overheating. The coolant gets heated by the leaking compression, and it's not a cheap fix.... very labor intensive. I'm not sure what a KA head will cost you, but I got a rebuilt head for my Mazda truck for about $250.... five years ago. My ride was also out of commission for over a week, while we took it apart and put it back together.

I'm sure there are others here that can answer with more detail. I'm not much of a mechanic, but this is what I've learned from my experience.
