A definitive guide to inspecting/installing tensioners?

I was looking into my current timing chain problem, which rattles pretty bad. I'm not sure if I need to go all out and replace the entire timing chain kit, or just a tensioner, so is there a guide out there explaining:

First, what to take out of the engine bay to be able to inspect or modify the timing system; And second, how to tell if the tensioners/guides are worn and need to be replaced, or if the chain itself is in need of replacement(without taking the chain off).
