'95 204sx Gear grinding

my 240sx will give me problems when going into mostly 1st and 2nd. In 1st i have to slow down to almost a stop befre i can put it into gear and sencond seems to catc even witht the clutch all the way down especially when it is cold. Also it is makig a humming noise like a bearing is bad and the stick has alot of vibration. my clutch feels soft and most of the time does not come all the way up but it does come off the floor.
