What hes talking about (if its correct) is the woodruff key going into the crankshaft...
Basically what this thing does is prevents the crank pulley from slipping on the crankshaft... On the crank pulley there is a little notch where this key slides into. They key is a half moon shape and sits in a groove in the crank...
Sounds to me like someone took the crank pulley off (either to replace the guides) and didnt get it on tight enough and the accessories caused it to shear it off... The problem is that the motor is probably going to need to be retimed, as well as the woodruff key replaced... I dont understand why they didnt replace it, if they know thats what the problem is.... IF its sheared off, all youd have to do is take a punch and tap the sheared piece out (carefully not dropping it into hte oil pan) and put in a new one.... ANOTHER problem i see with this, is if they didnt put the crank pulley back on it could mean its been starved for oil... The sprokets for the cams and oil pump are held in place by the force of the torque by the crank pullley. If it was too loose, that could mean the cams are off as well as the oil pressure being reduced...
There should never be enough force to shear the key off!!!! unless something else is off...
another cause could be the exhaust... If its a california model there was a TSB on the exhaust causing a vibration...
the way I see it, its not really that big of a deal, because you should always take the front cover off of a 90-93 to see if the guides have been done.
