2005 Quest, Idler Pulley is grinding

The adj. bolt broke on my 2005 quest, I figure this would be a simple change, I got the new adj bolt, a new pulley, and a new belt, I put it back together correctly from back(bolt/plate/washer/adjbolt/back plate,pulley, front plate,nut) That is how I took it off anyway. But no matter what I do the pulley keeps grinding into the adj. bolt, I already had to warranty it out once because it cut through the bolt. I have quadruple checked that all parts are correct according to the parts store.

What am I doing wrong?

Nvm, I got it figured out, turns out the spacer was screwed up, swapped it out and works great
