1994 has RPM specific rattle

Alright, so I'm hoping someone has an idea on this one, cuz I am fresh out.I've got a 1994 altima w/ 230K runs like a champ, but i have this light rattling/grinding noise which is present as I accelerate through the 2700-3000rpm range, and then again at about 5000rpm. Also, there is a rotational, but irregular rattle at idle, that sounds like it is coming from just under the valve cover near the front of the engine. I have had the cover off and have replaced the upper timing chain tensioner. Other than the noise Which doesn't seem to be getting any worse, the motor runs great. If anyone has seen this before, or has any ideas I would appreciate it.
