1993 240sx Clutch Pedal Bushings Question

My clutch pedal has been squeeking for about 2 years, and it got worse recently. Now everytime i clutch in the sound is a little louder and longer. It sounds almost exactly like those paper cutting boards at copy places like Kinkos. When i was checking around the clutch area, I saw a greased white object on my floor mat (see images below). I'm guessing a part of my clutch bushing broke off. I took some pictires of it and my clutch pedal assembly.

I searched around the forums and found out that there are 3 bushings, so i e-mail midway nissan asking for part #'s. I got this reply from mike.

"XXXX, When I'm looking at the pic, it looks like there are four altogether. 3 different ones, but one with a quantity of 2. Let me know. Mike J"

Whats the 4th one for?

the one in the pic is for the return spring. that's where the return spring rotates on the pedal bracket where the spring has the upside down U thing.
