I have an 86 turbo which does something similar.
If it has not been running for a while, say overnight, when I try starting it the first time, it turns over for 20 seconds or so, then finally starts, but runs really rough for a few seconds before smoothing out. I have found that if I turn it over for just a few seconds the first time, then turn it off for a couple seconds, then after that it starts right up - no roughness.
I am trying to profile the fuel pressure. I have a gauge in line and everything looks normal - it holds pressure for at least a couple hours after shutting down. It will drop down quite a bit over night. Odd thing though is that the pressure jumps right up (35 psi) upon first starting, but engine won't catch right away.
It has been doing this for years and hasn't gotten any worse - I just decided now it was time to see if I can diagnose it.
Other that that and one cold (sub freezing) startup surging issue, it runs fine. the above problem is not affected by temperature.
